Patient Portal Access

Our secure Patient Portal gives you the ability to access your patient information, request prescription refills, request appointments and complete required forms online. If you are already enrolled, simply select the “Existing Portal Users Login” below to access your portal account.

 

In order to enroll in our secure portal, you must first receive a PIN from our practice. Please contact our Medical Records Department at 252-757-2663 in order to enroll. Once you have obtained your PIN thenclick on the Patient Portal Registration below for instructions .

 

Patient Portal Registration Instructions

Patients looking to use our secure Patient Portal first need to request a PIN from our medical records department and then follow the steps to register their account. Click the button below for instructions on registering yourself, a child or dependent as a Patient Portal user.

 

Existing Portal Users

Patients who have already registered for the Patient Portal can access it here.

 

Enroll in Our Patient Portal

Please contact our Medical Records Department at 252-757-2663 to obtain a temporary PIN. If you enroll in our portal before receiving your PIN, you will need to contact our Medical Records Department to associate your account with our practice. After obtaining your PIN, please select the instructions from the following options. If you are registering a child, another dependent or a proxy, you will need to register yourself as a user first. I am the patient registering for myself.

 

Patient Portal Registration Information

Please contact our Medical Records Department if you have not received your PIN for registry with our Patient Portal. If you register without a PIN, you will not have access to your patient information. If you have received a PIN, please select from the following for specific instructions on how to complete your registration.

 

Option 1: Enroll Yourself in the Patient Portal If you’ve received your PIN from our Medical Records Department and are registering for yourself, select this option and follow the simple steps in the PDF document. View PDF Instructions

 

Option 2: Enroll A Dependent as a Parent/Guardian WITHOUT an Active Portal Account Select this option if you would like to enroll your child or dependent and you are a parent or guardian without current access to our Patient Portal. View PDF Instructions

 

Option 3: Enroll a Dependent as a Parent/Guardian WITH an Active Portal Account Select this option if you’re enrolling a child or dependent and you already have an account with our Patient Portal. View PDF Instructions

 

Option 4: Add an Adult Proxy to Your Account If you are the caretaker for an adult patient and are authorized to view their medical records, you can add the individual as a proxy to your account. View PDF Instructions

 

Option 5: Adding a PIN After You’ve Enrolled If you accidentally enrolled in the Patient Portal before receiving a PIN, you simply need to contact our Medical Records Department to request your PIN and then follow these simple instructions. View PDF Instructions
